“Rates of postoperative bleeding and transfusion are either widely variable or often unreported. As a high-volume center, we have anecdotally appreciated significant rates of each but admittedly have not accurately reported the degree of this specific morbidity in
our practice. This study aims to quantify postoperative blood loss, rates of blood transfusion, and the incidence of operative or endovascular intervention for bleeding, as well as associated risk factors contributing to this morbidity in patients undergoing open TAR.”

“In conclusion, TAR is a versatile technique for complex hernia defects but carries a significant risk for morbidity in terms of thromboembolic events and acute blood loss anemia. The use of therapeutic AC perioperatively is a risk factor for both reoperation and requiring transfusion. The ideal timing to resume therapeutic AC remains a challenging balance between bleeding and thrombosis. The findings of this study allow for a thorough discussion
with patients and their families regarding the morbidity of abdominal wall reconstruction in terms of postoperative hemorrhage. Providing comprehensive information will contribute to
patients’ informed decision-making.”
